Pentagon investigators determined that a February 28, 2026, U.S. missile strike in Minab, Iran, resulted from a cascade of preventable errors.

The strike killed 123 children.

U.S. officials attributed the tragedy to flawed intelligence, outdated imagery, and an overreliance on artificial intelligence.

The Pentagon used Palantir Technologies’ Maven Smart System to identify and select targets.

The Maven system fuses satellite imagery and drone feeds to accelerate targeting decisions.

Palantir has since updated the software with features to re-review intelligence and flag inaccuracies missed by human analysts.
