4DMedical's Record Scans Couldn't Stop a 26% Crash — Can A$242 Million in Cash Outrun a Widening Burn?

Shares in 4DMedical (ASX:4DX) suffered their worst single-session loss on record on October 30, plunging 25.61% to A$2.44 on volume of 18.5 million shares — roughly seven times the daily average — as investors digested the fine print of an October 28 quarterly update that mixed operational milestones with uncomfortable cash realities. October alone wiped 42.59% off the stock, and the year-to-date decline stands at 46.26%.

  • Record Scans, But the Cash Register Barely Rings. Revenue hit A$2.3 million for the September quarter, up 66% year-on-year, while a record 119,672 scans were processed, up 61%. Impressive growth — except operating cash outflow was A$15.2 million and investing outflow added another A$22.5 million , meaning the company spent roughly A$16 for every A$1 of revenue. Until that ratio narrows dramatically, each quarter drains the treasury faster than the top line can replenish it.

  • A Fully Drawn Loan Removes a Safety Net. The company's only debt facility — a A$10 million secured loan from Pro Medicus carrying a steep 12.5% interest rate and maturing in July 2027 — is fully drawn with no unused capacity remaining. Crucially, at maturity 4DMedical must repay the higher of A$12.5 million or up to A$20 million determined by its share-price performance , and up to 40 million shares may be issued to Pro Medicus around August 2027 — a dilution event that looms just nine months away.

  • A$242 Million Sounds Comfortable — Until You Do the Math. Cash fell A$35.2 million during the quarter to A$242.8 million.

Management estimates 16 quarters of funding at the current burn rate , but that assumes no acceleration in spending. Management has signalled it intends to invest aggressively in US expansion, which would accelerate outflows.

The company has already raised roughly A$233 million in placements during 2026 at prices between A$3.80 and A$5.90 — all now well above the current price, meaning those investors are underwater and future raises would be more dilutive per dollar raised.

  • Governance Friction Adds to the Overhang. Shareholders pushed back on remuneration-related proposals at the October 28 AGM, introducing a governance overhang that compounds funding anxieties. For a pre-profit company asking the market to trust its long runway, boardroom friction is poorly timed.

The core tension is stark: 4DMedical's lung-imaging technology is gaining clinical traction, but the gap between A$2.3 million in quarterly revenue and A$37.7 million in combined outflows demands either patience or more capital — and the market just signalled it's running short of both.
